Is the USMLE exam harder than the MCAT exam?
The USMLE is generally considered much harder than the MCAT due to its greater breadth of content, deeper level of clinical application, and sheer physical endurance required. While the MCAT tests foundational science and critical thinking over a single 7.5-hour day, the USMLE is a three-part series that culminates in Step 3, a grueling two-day, 16-hour assessment of a physician's ability to practice medicine independently. Furthermore, the USMLE requires mastering thousands of specialized medical concepts and pharmaceutical details that go far beyond the general biology and chemistry found on the MCAT.
Is Step 1 the hardest USMLE exam?
USMLE Step 1 is traditionally considered the most difficult to study for due to its massive focus on foundational basic sciences and rote memorization. While Step 1 has transitioned to pass/fail scoring, it maintains the lowest first-attempt pass rates among U.S. and international students for any of the USMLE exams, often serving as the steepest learning curve in medical education.
In contrast, many students find Step 2 CK harder to perform on, as it is now the primary scored metric used by residency programs, requiring nuanced clinical judgment rather than just fact recall. Step 3 is frequently cited as the least difficult in terms of content; however, its two-day, 16-hour format makes it the most grueling test of physical and mental endurance in the series.
How long is the USMLE Step 1 exam?
The USMLE Step 1 is a comprehensive, 8-hour examination consisting of up to 280 multiple-choice questions. In 2026, the exam moved to a more manageable format of fourteen 30-minute blocks, with each block containing a maximum of 20 questions.
The eight-hour Step 1 exam includes 55 minutes of total break time and a 5-minute optional tutorial. To optimize your time, finishing blocks early can provide extra break minutes to combat the mental fatigue during the exam.
Who is eligible for the USMLE Step 1 exam?
Eligibility for the USMLE Step 1 is restricted to students and graduates of accredited medical programs who meet specific institutional and regulatory criteria. To apply, you must be officially enrolled in, or a graduate of, a U.S. or Canadian medical school accredited by the Liaison Committee on Medical Education (LCME) for MD students or the Commission on Osteopathic College Accreditation (COCA) for DO students.
International Medical Graduates (IMGs) and students are eligible for the USMLE Step 1 if their medical school is listed in the World Directory of Medical Schools and meets the Educational Commission for Foreign Medical Graduates (ECFMG) accreditation requirements. Regardless of your location, you must have completed the basic medical science component of your curriculum and remain in good standing with your institution to maintain eligibility throughout your testing window.
